Roxane Laboratories, Inc. is conducting a voluntary recall of two lots of Sodium Polystyrene Sulfonate Suspension, USP, 15 g/60 mL Unit dose bottles. Sodium Polystyrene Sulfonate Suspension is used to treat hyperkalemia, an elevated blood level of potassium. The recall is being issued because of a possible yeast contamination, which could be harmful to patients with compromised immune systems. The affected lots are numbered 856396A and 856693A. Antipsychotic Drug Box Warning
The FDA issued a new requirement for all manufacturers of older, conventional antipsychotic drugs to incorporate a boxed warning indicating the increased risk of death when used to treat elderly dementia patients. Previously the FDA had required similar warnings for the newer atypical antipsychotic drugs, but recent research has shown that the older class of medication poses the same risk. Morphine Sulfate
ETHEX Corporation has voluntarily recalled specific lots of morphine sulfate 60 mg and 30 mg extended release tablets, due to the possible presence of oversized tablets containing twice the labeled amount of the drug. The recalled lots were distributed by ETHEX Corporation under an “ETHEX” label between June 2006 and May 2008. For complete recall details:

Digitek
Actavis Totowa LLC issued a recall of all strengths of Digitek pills, due to a possible risk of tablets double the normal thickness containing twice the approved level of the drug's active ingredient. Digitek is typically used to treat congestive heart failure and other heart problems, but a double dose can pose serious risk of digitalis toxicity, particularly for patients with renal failure. Digitalis toxicity can cause nausea, vomiting, dizziness, low blood pressure, cardiac instability, and reduced heart rate.
